Overview
Custom Forms can be used within Reviews to capture additional information about a learner. If your Centre has created forms but they are not appearing when a review is started, it means the Custom Form has not been assigned to the learner’s portfolio.
When an Assessor clicks Start on a Review, OneFile automatically displays all Custom Forms published for use in Reviews that are assigned to the learner at the moment the review is started.

Users who can start and complete Reviews will see Custom Forms, provided the forms have been assigned to the learner.
Steps
Why Forms Don’t Appear
If Custom Forms do not appear inside a Review, this is because:
- The form has not been assigned to the learner’s portfolio.
- The form was assigned after the review was already started.
- The form is not published for use in Reviews.
How to Fix the Issue
To ensure Custom Forms appear inside a Review, assign the form to the learner before starting the review.

Tips & Notes
- Forms must be assigned to the learner before the Review is started.
- If a review is already in progress, the form will not appear until a new review is started.
- Ensure the form is published and enabled for use in Reviews.
