Overview
Learner Template Forms allow you to record multiple instances of the same form for a learner. Each instance can be completed and signed independently, making them ideal for repeated activities such as visit logs, workplace evidence, or recurring reviews.
These forms also allow signatures to be captured every time a new instance is completed.
Who Can Use This Feature?
Any user with permission to complete the first required signature on the form.
Centre Settings
No additional centre settings are required to use Learner Template Forms.
Steps to Create a Learner Template Form Instance
Step 1: Navigate to the Learner’s Portfolio
Step 2: Open the Forms Section
- Click the Forms tab in the Information & Options area.
Step 3: Select the Form
- Click the form Title.
Step 4: Create a New Instance
- Click Create New Instance.
- (Optional) Rename the instance.
- Click Create.
Completing the Form
- The form can only be created or completed by the user who is assigned as the first signature on the form.
- If the option to create a new instance is not visible, it is likely that the form is set up to be completed by a different user role.
- Contact a Centre Manager to confirm form permissions if required.
✅ Tip: Give each instance a clear and unique name (e.g., “Visit 1 – Jan 2026”) for easier tracking and reporting.
