Create a New Form Template

Overview

Form Templates allow Training Providers to create custom forms used throughout the Enrol module. Forms can collect learner or employer information, capture signatures, or support compliance and onboarding requirements.

User Roles

This feature can be used by the following role:

  • Training Provider

Steps

Create a Form and Add Fields

  1. Click Form Templates in the navigation menu.
  2. Click New Form Template.
  3. Select the Form Type.
  4. Click Next.
  5. Select who will complete the form (e.g., Learner, Employer, Training Provider).
  6. Click Next.
  7. To add fields, click the icon for the field type you want to add.

Field Types and Descriptions

Field TypeDescription
HeaderDisplays an image (e.g., company logo) at the top of each page.
Single-line textOne line of text that does not expand in size.
Multi-line textAllows several lines of text that expand automatically.
NumberNumeric input field.
DateSelect a date from a calendar.
Single checkboxA single checkbox with a statement.
Multiple checkboxDisplays several checkbox options.
Radio buttonSelect one option from multiple immediately visible choices.
DropdownSelect one option from a dropdown list.
SignatureCaptures a printed name and digital signature.
TitleStatic single-line text displayed as a title.
ParagraphA section of static text.
Data tableInserts a data table with input or read‑only modes.
FooterDisplays an image (e.g., accreditation or quality logos) at the bottom of each page.
HyperlinkAdds a clickable link to an external source.

Additional Options

  • You can change field order, edit fields, or delete fields by hovering over them.
  • You can add prepopulated fields that pull data automatically from Learner or Employer records.
  • You can save the form as a draft at any time and return later to complete it.

Publishing Your Form Template

Click Publish to make the Form Template available to your Applicants. Once published, the Form Template cannot be edited.
