User Roles
This feature can be used by the following user roles:
- Training Provider
To create a new Form:
- Click Form templates in the navigation menu
- Click New form template
- Select Form type
- Click Next
- Select who will complete the form
- Click Next
To add fields, click the icon of the field you'd like to add to each section. A description of each field type is displayed below:
- Field Type
- Description
- Header
- An image, typically your company logo, displayed at the top of each page
- Single-line text
- One line of text that doesn’t increase in size
- Multi-line text
- Several lines of text that automatically increases in size
- Number
- A numerical integer
- Date
- Select a date from a calendar
- Single Checkbox
- A single check box with a statement
- Multiple checkbox
- Options that are presented as checkboxes
- Radio Button
- Select one of multiple instantly visible options
- Dropdown
- Select one of multiple options from a drop down list
- Signature
- Capture the name and signature
- Title
- Add single line of static text, displayed as a title
- Paragraph
- A block of static text
- Data table
- Insert data table in read only or input mode
- Footer
- An image, typically accreditations and quality logos, displayed at the bottom of each page
- Hyperlink
- Link to external source
You can change the order of the fields, edit or delete the fields by hovering over the field.
You can also add prepopulated fields which will pull the selected data from the Learner or Employer record.
You can save the form to draft at any point, if you wish to finish later.
Publishing your Form Template
Simply click Publish to assign the Form Template to you're Applicants. Once the form is publish it cannot be edited again.
