Create a New Form Template
Overview
Form Templates allow Training Providers to create custom forms used throughout the Enrol module. Forms can collect learner or employer information, capture signatures, or support compliance and onboarding requirements.

This feature can be used by the following role:

Steps
Create a Form and Add Fields
- Click Form Templates in the navigation menu.
- Click New Form Template.
- Select the Form Type.
- Click Next.
- Select who will complete the form (e.g., Learner, Employer, Training Provider).
- Click Next.
- To add fields, click the icon for the field type you want to add.
Field Types and Descriptions
| Field Type | Description |
|---|---|
| Header | Displays an image (e.g., company logo) at the top of each page. |
| Single-line text | One line of text that does not expand in size. |
| Multi-line text | Allows several lines of text that expand automatically. |
| Number | Numeric input field. |
| Date | Select a date from a calendar. |
| Single checkbox | A single checkbox with a statement. |
| Multiple checkbox | Displays several checkbox options. |
| Radio button | Select one option from multiple immediately visible choices. |
| Dropdown | Select one option from a dropdown list. |
| Signature | Captures a printed name and digital signature. |
| Title | Static single-line text displayed as a title. |
| Paragraph | A section of static text. |
| Data table | Inserts a data table with input or read‑only modes. |
| Footer | Displays an image (e.g., accreditation or quality logos) at the bottom of each page. |
| Hyperlink | Adds a clickable link to an external source. |
Additional Options
- You can change field order, edit fields, or delete fields by hovering over them.
- You can add prepopulated fields that pull data automatically from Learner or Employer records.
- You can save the form as a draft at any time and return later to complete it.
Publishing Your Form Template
Click Publish to make the Form Template available to your Applicants. Once published, the Form Template cannot be edited.
